FK Slavika was a Dutch team from Fryslân. The team was founded on the 8th of December 2003 by Userper under the name of Slavika in the Dutch VIII.809. The team changed its name to FK Slavika during the break between season 13 and 14. Before stopping to participate actively, the team managed to climb up to division V.
